Abstract: 

The relation between longetivity of toal knee replacement and alignment are measured in all its parameters. Alingment in 3° has been comprehensively accepted on the basis of limited number of outcome studies. However this alignment constraint still remains a hypothesis, as 3° is an arbitrary number. The aim of this study was to understand the load distribution in alignment range of ±3° and more. The experimental set up was designed and fabricated to facilitate the coronal plane alignment in the range of 7° varus and valgus malalignment. Load distribution was observed on display in medial and lateral compartment at increment of 1° in varus and valgus. It was found that the rise in load within ±3° in medial and lateral compartment were not much, however beyond ±4° there was gradual increase in load and load imbalance at every incremental change. This investigation supports the need for precise construction of load bearing axis in coronal plane and validate the clinical outcome for acceptance range of ±3°. These results also demonstrate that there is less percentage chage in valgus deformity than varus.
